Maybe I shouldn't have included the note about the neutral. POCO did not pull a neutral conductor underground from the pole. Not sure where they thought it should be bonded to ground- so the origin of the neutral connection is actually up in the air until I get back with the engineer.
POCO must pull a neutral, a ground rod will not have low enough resistance to provide you a reliable neutral path.

You are absolutely right and that was what had me puzzled as to how they did the underground to the meter base. I had been looking at the top of the riser on the pole where they had left the wires temporarily taped off to the side of the conduit- I only saw the two hots. Went back out to the jobsite yesterday and opened up the meter base and sure enough- there was the neutral conductor after all. I took another look at the pole and could then see where the wire was just hidden behind the other two wires where they had taped it off to the side of the riser. Mystery solved.

So the inspector is good with the plan- 2 wire runs to two 200A panels. Conduit fill is fine based on Table C.11 for (4) 250 kcmils and (2) 4/0 THHN/THWN- it actually leaves us plenty of room for larger if need be, even accounting for voltage drop and derating for multiple runs in the conduit if need be. I need to confirm the heating packages then run it back through the load calc., but I expect that we are good and it will be nice to move forward on this.

Thanks so much for everyone's help- hope I never have to do another service like this. During my apprenticeship (20 years ago) my journeyman was the always the one to handle all of the service stuff (which was all almost exclusively 200A res) , so I have had to do quite a bit of review and that's why I really appreciate the help.
