300.4 (F) Insulated Fittings. Where raceways containing ungrounded conductors 4 AWG or larger enter a cabinet, box enclosure, or raceway, the conductors shall be protected by a substantial fitting providing a smoothly rounded insulating surface, unless the conductors are separated from the fitting or raceway by substantial insulating material that is securely fastened in place.

I would say that an offset nipple with conductors smaller than #4 would not require an insulating bushing, #4 or larger would.

I would like to add that, around here, it has been allowed that the sheath of service cables qualifies as the "substantial insulating material that is securely fastened in place" when running service cable through nipples.

I have one inspector that insists on a 2 in bushing on a 2 in nm connector when 4/0 AL SER is used I know it is not required in the NEC but this guy will pass inspections for me with a call telling me to add a receptacle and he marked where the rough crew missed spacing,or wrong breaker size has beeen installed even have had him call me and say on a final light(s) have not been installed.I tell him _ _ _ _ I will have it taken care of today and he signs it off and yes he has called me to have a 2 in bushing installed on 2 in nm connector :D .Sometimes I rock it so hard I don`t know how it stays afloat,but at these times I have my facts and ducks lined up :D
