I have a customer installing "smoking rooms" 10' off the existing building and they have installed a 480 volt single phase infrared heater (outdoor rated.) The room is basically an ATV shelter all metal with a roof and no sides. Do I need a ground fault circuit to feed the heater?

No. Think of it this way. How much 480 volt equipment gets installed outdoors? A lot! Roof top HVAC units, lighting, motors, to name a few. None of that needs GFCI protection. All that's important is that the heater is properly grounded (bonded).
Generally speaking, GFCI protection is only required for 15 and 20 ampere receptacles. There are a few exceptions to that but for the most part it applies to receptacles.
