5kV cutouts for transition from Hendrix to underground

Can anyone tell me where I would find minimum distance between phases on a cross arm with 3 5kV fused cutouts (with integral arresters)? Is this information in the NESC and if so could anyone point me to it.

After having looked in the NESC for the answer to my own question above, I see that Table 235.2 may well be exactly what I need. Yet, I cannot for the life of me find this table in the NESC. Can anyone help?
