6-wire service?

Anyone here familiar with what we call a 6-wire service? The utility I work for has some old services where customers required both a single phase 120/240V and a 3 phase 3-wire delta service. The company only allowed one service point so they had both of the drops come into a meter enclosure and installed a 6-wire meter. I ran across one of them today when one of our meter technicians called. He was testing the meter due to a commerce commission compliance audit but ran into an issue. The power bank was grounded and in effect caused a high leg on one of the phases. I am looking for any information with these services so that I can understand them better and how to convert them when needed. Thank you in advance for any help you may give me.
 
Most likely just a high leg delta. Heck, it could even be a high-leg open delta service with stinger and lighter pots. I'm going to hold out for pictures of this so-called 6-wire meter before drawing any rock solid conclusions.

Yep. 3 xfmrs wired closed delta with one that had a center-tapped neutral (or 2 in open delta) could accomplish what the OP describes with 3 ungrounded and a neutral. Neutral isnt going thru the meter, so 3 hots.. or 6 wires if double tapped from the xfmrs.

There is a good chance the OP has a CT meter which has more conductors than average mistaking that as the metering of two services.

A three phase meter would have six wires, three line three load. It may or may not have a seventh jaw for a neutral conductor.

If you are supplying both 240 volts three phase as well as 120/240 single phase directly from the same source you don't have much choice - it will be a high leg delta system.
