(5) "....... Secured within or under the perimeter surface 4 to 6" below sub-grade."

Inspector failed out grounding ring around the pool as it is approx. 14" below sub grade. SECURED to 4 points of the pool 4" below sub-grade.

Our Service Dept. has installed 136 pools or hot tubs in the last 1.75 years with NO failed inspections.

Have we been installing the ground ring wrong?

Maybe I’m missing something? Sounds like you’ve already answered your own question? If you’ve been installing it 14” deep, it’s required to be 4-6”. Maybe a little more detail to clarify.
