well maybe he should be more careful, or carry an extension cord instead of rigging the disconnect.
My point is, you cant stop stupidity. And you can only minimize accidents. Why doesnt the code enforce breaker locks for EVERY breaker in the panel? Why ONLY the equipment??
And about money: I couldnt care less about $100 to add a service outlet. I would prefer to spend that time setting up an industrial machine, which is worth $1000.
