AC Motor Voltages

We are qualifying equipment in our plant and tested an AC Motor stamped 115VAC. The house supply voltage is 208VAC. The motor electrical plug was also 208VAC type. The motor was wired such that two of the phases were not landed, thus supplying 115VAC, single phase through a 208VAC electrical plug. The motor has been in operation for years and runs under load far below full load amperage. Does this configuration meet NEC code?

Where is the other motor lead connected, a grounded conductor or a grounding conductor?

If the outlet is a 208/120 4 pole 5 wire you are probably all set.
