Is ground fault protection required on the next downstream breaker for panels after the main in hospitals? IE... secondary breakers downstream of the GFI protected main in a switchboard.

Yes, if the main breaker (i.e. one on the service) in a hospital has GFCI, the next set of breakers downstream must have GFCI. The idea is that a fault on one of the feeders is less likely to shut down the whole hospital. See 517.17.

Steve, you are talking about GFIs, not GFCIs. I know you know that but not everyone knows the difference. A GFI is for protection of equipment and a GFCI is for protection of people. :D
