I am confused on when things need to be updated and when they don't. If you have a receptacle in the bathroom of a house that isn't GFCI, when the receptacle needs to be replaced are you required to replace it with a GFCI or can you go back with a regular duplex?

Annex G is not enforceable unless the jurisdiction you work in adopts the Article. I am not aware of single state or area in the US that has.

Fortunately, your specific question has a code answer. See section 406.3(D)(2). A gfci replacement is required.
