Anyone know of an in-floor junction box? Located in kitchen.

How many conduits of what sizes and fill?

I would look at every way to not have to do this, barring that here's what I would do:
Consult structural if applicable, confirm not a PT slab, get xray, then cut a clean square hole in the slab around the conduits. Augur a big hole a few feet deep in the bottom and fill with drainage gravel. Using stainless fasteners, bolt stainless angle iron into the sides of the concrete 1/4" below the surface of the slab. Get a cover plate the size of your cutout fabbed from 1/4" plate stainless with countersunk holes around its perimeter. Set the plate in the hole and transfer mark your angle iron, drill and tap the angle iron for the mounting screws that hold the cover down. Install regular junction boxes in your new vault.
 
Another option similar to what Todd0x1 has mentioned would be to use a "floor door" such as the one at the link below to provide access to regular junction boxes. The company has many different types of access doors depending on what you want.

Those look good. Alot easier than what I proposed. Mine gets you completely flush with the existing floor though.

It might be possible to cut away the tile under the flange of the floor door so that it rests directly on the concrete, allowing the door to be nearly flush with the tile surface.

Lots of good ideas…. The problem with custom stuff is the clock is working against us. It’s not a PT slab, I know that for a fact.

Likely will come to a decision tomorrow on how to proceed.

This is a good option, had not considered this sort of install.

This isn’t the first time this sort of install has happened to me, and won’t be the last, I was looking for a quicker fix. Even if we don’t use this now I’m going to work on a solution to have in the future
