It's not explicitly stated, but I think it means the AHJ. This statement is giving the AHJ the authority to approve other means than are listed in (1) through (4).

Look at Article 100 05' NEC "Approved" Yup! AHJ and That can be scarry. Ran a service call about 3 months ago and could not help but to notice 5 feet of "white pipe" stubbed up from grade to meter was 2" schedule 20 DWV Plumbing pipe. Recent service upgrade and was signed off and "Approved" by a local AHJ.
