Is there a code out there that requires a main breaker of fuse to be located on the load side of an ATS (similar to the 10 foot tap rule requirements for a transformer secondary). Is it okay to terminate the feeder directly into an MLO 42 space panelboard on the load side of the ATS?

If this generator is installed on the outside of the building it is serving then there will need to be two disconnects on the LINE side of the transfer switch unless the transfer is rated as service equipment.

On the load side of the transfer switch it could be what ever the installer designs that would be within code for any other installation.
 
I agree, but would like to clairify: if there is a circuit breaker somewhere upstream of both the normal and emergency inputs, you don't need another main on the load side. A main breaker on the load side would make the ATS usless if it tripped.
