I am trying to fill in the banks on the free Mike Holt Available Faut Current Calculator and can not figure out which voltage to use on the voltage option. The electrical system I am designing is 120v/240v 3 phase (High leg). Do I use the 120/208 option?

I see some Canadian and mexican voltages also. Odd, no 240 delta...

No. There is no option for a high-leg delta.

That's a good question. Not sure how close it would be if you used 120/208-- maybe Jim can expand on his comment some more.
You might could piece together something with some iterations, but I would not use the 120/208 alone. By the time you made the correct adjustments, you would be just as well to calculate it yourself without the free tool. Just follow the free Bussmann guide.
