Bonding without sepearte EGC in subpanel

My home was built in 1994. The utility pole is 70' from my house. At the main panel at the pole the ground and neutral are bonded. A 150A breaker feeds a subpanel at the house through 3-4/0 AL conductors running underground without conduit. At the supanel in the house the the neutral and ground are bonded. A #4 bare copper wire runs from the neutral bar to a ground electrode. Should the ground and neutral be isolated? Does a seperate EGC need to be ran back to the main panel.
