Basically this is the situation, old residential building, old wiring that is a fire hazard, the owner doesn't want to do major renovations so we going to abandon the old wiring and run new surface mounted conduits with new outlets and light fixtures i know its ugly but this is what the owner wants. Now in one of the rooms in the apartment there is a ceiling fan that is mounted to the box that is mounted to the beam in the ceiling, so can i mount an extension box to the existing box and then mount the fan on the extension ? :)

You can't use a standard extension box. They are not fan rated. You can use a Wiremold fan box mounted over the existing box.

As Dennis stated you can just support the fan independent of the box and you can use a standard box with an extension box.

Which makes hanging the fan legal, but does not, IMHO, get you immunity from the requirement to use a fan rated box in new installations.
How this is resolved in remodel/rewire situations may depend on how your AHJ looks at it.
