Yes you can replace the riser and service conductors without doing the panel. But inspection is Better answered by your AHJ.

Possibly, depends on local POCO requirements. Here, if an old "A-Frame" meter pan is existing, I'm required to replace it, along with any old cloth covered SEU. I would also be required to bring service grounding and bonding up to present day code.

Around these parts anytime you do work on a service the entire service will need to be bought up to current code(s) including service drops, conduit mast, feeders, meter can, grounding/bonding and panel and on and on and on.

You could get a hundred different answers on this one from one hundred different locations.

The answer lies in the rules of the AHJ where the work is to be done, and possibly some rules from the serving utility company.

Contact both if you are not sure what applies to your situation. Sometimes there is a difference depending on if it is seen as a repair vs some attempt at improvement as well.
