Can You Spot The Issues/Violations

The total lack of respect for the NEC, safe working practices, and the list goes on and on.

These pictures are taken very close to the switch, I was wearing PPE the camera was not. I utilized a 12-24MM lens at 12MM so there is some image distortion. I could address this is Photoshop, but didn't.......SORRY.

Let me see..
1. The secondary of the CT is open (CT is not visible) two conducors on bottom right, black and white twisted pair.
2. No way to open the bolted pressure switch, the CBC (Charge Before Closing)operator has been removed. Not that the switch it is closed.
3. No fuses, oh wait the 4/0 is serving in place of the 3000 amp fuses.
4. No GFP, avoids nusinace tripping and the hassles involved with that.

from my inspection the CBC was laying on the floor in front of the switch, the electrician tried to open the switch with the handle in lieu of the little red button that says PUSH HERE. Broke the operator and removed all parts CYAing his self. The building owner had not a clue this issue existed from a previous installation some years ago.
 
the electrician that broke the bolted pressure switch obviously didnt know how to operate the CBC if he couldnt push a button. thats pretty horrible to see that kind of hack work on something like that.

do you think an electrician done that work or the maintanence department?
 
At present I have 4 bolted pressure switches with operator error issues, in the process of repairing or replacing as necessary.
 
They easily fit in if you can lift those puppies. I just had to replace some 6000 amp fuses with 5000 amp (electrician installed the wrong amperage fuses) talk about heavy and expensive $1,525.00 each.

Maybe thats why this guy put in the 4/0 , he couldn't afford the fuses.:-?
I can't even begin to imagine what would cause someone to think that was acceptable.
 
From what I have gathered, There was an operation of the GFP, the contractor was called and either the CBC was defective or as is often the case he did not read the instructions and broke the CBC operator.

To restore power the switch the operator was removed, I have no clue how the lack of fuses fit into the picture. BUT the contractor told the owner all was fixed and received payment. This was over 4 years ago and was recently discovered by another contractor. This contractor told the owner to contact me. I have parts and fuses on order.

These are good pictures.

Whether the installer/technician was paid or not, this is a criminal act.

Good for the contractor who had the guts to contact the owner/Brian to correct this mess.

It never ceases to amaze me how much work is performed by companies/individuals who are not aware of how their ignorance/stupidity can create such hazards...with little regard for other people's safety.


The only good consequence of work performed like this, is it provides Brian and companies like Brian's some more work.

I am willing to bet all of my money (I rarely bet 'my' money) that Brian will properly perform this installation whether or not he is paid ('stiffed') or not!!!
