I have seem this also numerous times and always thought it was a violation.Exactly, and usually the fourth circuit is from a different isolated ground panel or fed from a UPS/UPC panel.
The isolated ground is still a ground common to the other ground, it just does not rely on the conduit as a return path. If properly installed, both panel grounds are bonded. I have seen many times, the isolated ground incorrectly installed, and truly insulated back at the panel.I have seem this also numerous times and always thought it was a violation.

Don’t say you could bring a third g wire from source 2, because the factory whips have 8 wires.
If Source 2 plug hot wire faults to case…does circuit clear ?
