For those that this applies to, Jusisdictions that adapt the 2012 International Residential Code and don't amend out the electrical sections, Section 4003.11 will apply to fixtures located in the "BATHTUB ZONE". Ceiling fans that have blades that infringe into this zone also need to be relocated.
Chandelier over a tub was one of my most aggravating jobs when I worked with my last boss. 2nd floor bathroom, about a 16 foot ceiling. 2 of us had to get an extension ladder up a spiral stairway and maneuver it around corners, into bath and up to ceiling. But at least then, people who wanted crazy things would pay for them. See my new post on such issues.
One customer back in the 80's when I was still learning the trade ask me to install a chandelier above his tub. I suggested to install a battery operated light, he agreed and told me that my idea is really bright but can't use me because I already solve the problem for him. He said his flash light will do the job better with zero expenses.
