Coax cable caught fire

Is the cable box's chassis ground and its power cord's neutral connected? How else could this happen?

I don't think I've ever seen a cable box or cable modem that has a three-wire line cord. I have to assume that it's listed as grounded through the coax shield.

Probably not much anymore with the flat screens, but you used to see some older TVs with a hot chassis usually due to the internal power line filter components failing. That tended to put some leakage voltage on the "F" connector which caused a spark or a shock when you connected the coax from the cable box.
