That really doesn't look like poured concrete. It looks more like cinder blocks but we know that can't be the case. LOL

You can actually surface mount a pancake box which will be more shallow than the wiremold box. There also looks like there is plenty of concrete to cut in a pancake box. If the fixture will allow the 1/2" then just surface mount it.

More than likey it's a precast concrete slab.
Although the texture does look like a cinder block
 
The texture looks like spray-applied texture, and so reveals nothing about the underlying material. You can see the underlying concrete around the threaded inserts.

I know it's a slab.
If you look beyond the textured paint it concrete looks kinda of porous.
He could chip it out to recess a box but he'd have to get creative in securing it.

Its an old 5 story building. This is on the 1st floor above the garage. Anyway.. here's a close up if this helps confirm you guys thoughts. About 1.25" thick

I thought about recessing but might be too risky

What is on the other side of the concrete? Someone else's floor, an accessible concealed space, or a void? I'd use the inserts, but if something gives its nice to know your options should you need to recess the box or drill all the way through.

Theres a small void then the floor for units above
