What code or standard provides for the doors of a switchgear enclosure to contain the arc-blast associated with a major fault (such as phase-to-ground) within the enclosure?

An arc rated switchgear might direct a blast upward (not contain) when the doors are closed, but how often is there an arc blast when the doors are closed?
I think that arc rated switchgear is not worth it, as many accidents happen when the doors are open.
Just my opinion.
