The 12" refers to romex going into boxes as per 2008 NEC 334.30.

Or MC 330.30(B), most of the time 12" applies, unless the MC is being used in accessible ceiling as described in...

In the case of prefab luminaire whips, does there need to be a support within 12" of the box before the 2x4 (i.e. does the whip have to be supported at the 4 square?

Not if you use an MC fitting. Type MC cable fittings shall be permitted as a means of cable support. Connect it and call it supported.

I wonder why an MC connector is considered good enough to be a means of support in an accesible ceiling (not exceeding 6' respectively) and not in wall? Your telling me an electrician will periodically come by and poke his head up in the ceiling to make sure it is indeed still strapped if its accessable? Or maybe its just darn near impossible to strap it accordingly in those types of installations? It seems to me being in a concealed wall would be a safer installation using the connector as a strap than up in, say a dropped ceiling were personel could actually pull the cable loose later on while installing data, or lord knows what else.

Just wondering

Yes, good call. I suppose it would create a problem if all your MC's are flappin' in the wind by the connectors while the drywall is going up.
