Typically it refers to the bushings. I would assume a dead front transformer to have elbow connectors on the high side.
A live front uses terminators on the ends of the high voltage cable and has an exposed, bolted connection at the insulators inside the cabinet.
A dead front uses load break elbows to make the HV connections to the TS.
Both have exposed secondary.
