Is the difference between service disconnect and meter disconnect that fact that one is served rated and the other isnt? They both isolate premise wiring from utility wiring when off and are both on the load side of the meter, correct?
 
The meter disconnect is something that is required by the utility and is on the line side of the meter. It is to make it safe for utility workers to install and/or remove meters. In my area, a meter disconnect switch is only required for self-contained 480 volt meters.
 
Locally, meter disconnects, when required, are locked and controlled by and controlled by the utility. The property owner/manager can not turn it off.

Also, they aren't technically covered by the NEC. They are usually non-fused and you don't have an equipment ground in the load side.
