Do You Charge Extra For Going Above and Beyond?

I was doing some change orders today, relocating recessed lights. The job involved, of course, crawling around the attic with blown insulation. The company I work has the usual hourly rates but we do charge extra for jobs in certain locations, mainly when working at waste water plants and related. It made me wonder if digging through insulation should warrant a higher charge than, say, moving devices or changing lights, etc. I have always considered this part of the job, but it can affect the rest of your work day and even what you do afterwards. Do you charge more for certain jobs or do you feel it all balances out in the end?
 
I would charge extra for crawling in attic and crawls, but that is just my opion. I wouldn't kill the customer either.

If I know it is going to be difficult. Yes I would charge more.
Some times something you bid on will be easier. Some times harder.
Insulation sucks. That's what helpers are for.
