In the Mike Holt's site I encounter a document called "Electrical Plan Review: Calculations Answer Sheet" (source: Cooper-Bussman with 2 pages).

Question: ?Where I can find the complete document?

Could it be the one titled:
"Selective Coordination Checklist"
You might have to load several and check them visually.
