Ungrounded wire colors are, by and large, not really an NEC issue.

But I did notice a lot of the numbers on the wires don't match up with the breakers numbers (which don't follow the 'ordinary' numbering methods).

The issue is with the breaker lug not the panel. The wire capacity is stamped on the side of the QO breaker, they (10-30A) have been rated for two conductors for about forty years.
Same with C/H.

That's the most serious possible problem with what HI's call a "double tap."

Its pretty ovious when a termanal or breaker is rated for two wires, When youve been in the trade a while this will become more clear as it has for me..
 
You have to be careful and make sure that the terminal is listed for more than one conductor per section 110.14 (A). It may look obvious but it may not be listed .
