Dryer GFCI breaker tripping

I ran a 4 wire circuit for a dryer with a 30 amp double pole GFCI breaker. The appliance people delivered and installed the equipment. The circuit breaker trips when I attempt to reset it.

If the installers failed to separate the neutral and ground, would that cuase this problem?
 
It still amazes me how often dryers and ranges get miss-wired even when there is a sticker next to the wiring compartment that shows how to make the connections.
 
Delivery drivers don't read labels.
Truth. They also don't bother to check whether or not the builder punched out the washing machine drain before attempting to run a test wash.

(is naming and shaming the parties involved against board rules? though admittedly this happened roughly 20 years ago)
