Section 220, examples in Section 220, and the dwelling unit calculation examples in Annex D do not indicate that a washer load be considered in the dwelling unit calcs. Is this not required or is the washer load covered within the laundry or (2) small appliance branch circuit loads?
 
Even though the code does not explicitly say so, the required "laundry circuit," with its assigned load of 1500 VA, is generally understood to be intended to serve the washing machine.

Yup.

The washer, and gas dryer, together are covered by the "laundry circuit" designation as 1500 VA.
