Looks like it is always required. And has to be within sight and 25' of the EVSE equipment.
West coast does this a lot.
It's a royal pain, and does not scale well at all with multifamily garages.
Local folks have been avoiding doing projects in the cities that have added this requirement, or been burned by it.

The most frequent nominal reason cited is that the labels in the electrical rooms are a mess, and
that the EMS are not able to otherwise determine if an EVSE is on or off.
