NEC 690.13(F) (1) calls for externally operable general-use switch or circuit breaker, or other approved means.

Anyone familiar with the definition of externally operable?
Is this the OSHA definition or NEC has another definition?

I've used a shunt trip with an external push-button and for businesses or industrial in a KNOX box for the FD.

The NEC defines it in article 100, and it just means the phrase that follows your 690 reference: "without exposing the operator to contact with live parts."

I suppose any circuit breaker or disconnect with a handle is ok, but a pull-out type disconnect might not be.

Scratch that, partially. They specifically say a load break pused pull out is okay. But I guess an unfused one wouldn't be?
