Fiber in conduit

I'm going to run single mode fiber (2 strands) about 1000 feet. I plan to use 1.25 PVC conduit and bury it. How often would it be prudent to put pulls? Can I just use inline pvc junction boxes?

If I want a riser about halfway where I want to add a run of RG6 coax and 3 #12 conductors, what's the best way to do that?
