Floor drain in electrical vault?

We have water coming into an electrical vault, our solution is to install a floor drain. Question, is this allowed and if so would a flush mount metal drain cover need to be outside the 3-4ft distance defined in 110.26 working clearances? I'm concerned about exposed live parts clearance to grounded parts.

The dimensions in 110.26 are for vertical grounded parts. The entire floor could be made of metal. Also concrete is considered as grounded.

Does the vault contain any oil filled equipment such as a transformer?
