Forklift Charger Power

i havent been to the building so i couldnt check the panel myself but if i have two hot wires reading 120v each and together reading 212v but also a third wire reading zero by itself what kind of circuit is this? or is the third leg dead?
the power is for a forklift battery charger that can run in 208v or 240v depending on supply. im not comfortable in my knowledge of three phase power enough to troubleshoot so i thought id ask you guys. thanks for your help in advance!
by the way these readings are from the receptacle, not the source panel.

It sounds like 120/208v 1ph (two poles) from a 3ph panel.

Do you get 120v or 0v from either 120v terminal to the "dead" terminal?
 
Sounds like @HEXx is describing measurements made by someone else.

1) A single point can't have voltage, it must be measured relative to something else.

When you say '2 hot wires reading 120V each', that is relative to what? The third wire or a grounded enclosure?

When you say a 'third wire reading zero by itself', same question, relative to what? Is the wire colored?

2) With 3 phase power loads can be served by 3 hot wires (all 3 phases), or by 2 hots and a neutral, or just by 2 hots, or by 1 hot and the neutral. You need to know what the load expects. A forklift charger could use any of these arrangements. You need the nameplate data.
