Why do these lugs always have to be so sharp???
I carry a roll of thick scotchkote tape that I put on the offending edges in situations like this. 2 layers + 33 on top pads the edge plenty. Another option is 1-2 pieces of heat shrink on the vulnerable wires. Even this may not last 40 years though, if the wires move around a lot due to motor starting and inrush, they may chafe very quickly. The worst is if they don't short, and somebody comes along and moves the wire just enough...and things blow up in his face

Better yet, train/retrain the conductors...

Otherwise jam in a 2x4 between the conductor and lug... ;-)

What wood works best? I prefer oak... ;-)

What happend to the buss in the bottom picture? Looks like it has been drilled or something. Is that the problem you are referring to?
No, the issue is shown in the enlargement: the upper bus's right-side conductor is pressing against the middle bus's left-side lug.

The conductors should have been bent in such a way that there should have been space netween these surfaces.

I vote that the installer should have to go back on his own time, and with the equipment energized!

No excuse. Boo! Hiss!

Added: I carry a couple of pieces of PVC to use as bending handles. Slide one over the wire, bend, slide up, bend again, etc.

First I want to apologize for being a slacker. I posted this on another site. Lots of reasons for my slacker mentality. I have been busy, on the phone, kids, dog ate the picture (DIGITAL?) ok just plain lazy.

This is a bus tap for an emergency panel, #2 AWG, 1000 amp switchboard, with provisions for a bus tap, few issues though.

1. The conductor termination connectors are two small for the mounting
holes in the bus, so they put steel washers behind the termination
connectors.
2. The conductors enter the termination connectors from the top.
3. The tap was somewhere between 40-50 feet to the OCP.
