my question is with generators in health care facilities, in the nec i was told that you have to have a on site fuel source, does that mean you cannot have natural gas?

Maybe yes, maybe no.

Look at Section 700.12(B) and the exception.

Basically it is up to the AHJ.

Be careful here, there are a lot more documents than just the NEC that govern health care facilities.
