We have urgent care in tenant fit out that has procedure rooms, exam rooms, and x ray room.

The urgent care are thru out USA. So its not one doctor private practice. Doctors are hired and they come and go.

In such as place would Chief Operating Officer be considered as governing body for purposes of providing letter regarding patient care space etc?

In TN the governing body is the The Board for Licensing Health Care Facilities. I believe you will find the health care governing body in most States to be a governing agency, State or Local

I have never heard of that. Usually governing body NEC 2017 section 517.2 definition of governing body is doctor but in this case its not private practice.

Look in your state registry for who governs health care facilities, most states if not all have their own agency.
