Is a ground required form the electrical service entrance to the sub panel if the sub panel is the first point of distribution?
Ground between electrical entrance and sub-panel.

There has to be a Service Disconnect. Everything after the SD needs an Equipment Grounding Conductor.

To answer what seems to be the question, if the "service entrance" means a main breaker, yes, you need a 4-wire feeder and non-bonded neutral in the distribution panel. Is this a modification to an existing installation or is it new work?
