is the ground rod required to be installed in the exterior of a building even if the service disconnect is inside?

No, the requirement is that the rod be in contact with 8' of dirt. Doesn't matter where that dirt is. I've heard some arguments about permanent moisture but many commercial buildings have the rods installed below the basement slab with a ground rod test well.
