I am wondering when do grounding bushings need to be used.

Not as often as many people think. Take a look at 250.90-100 or so. Notice the section for "bonding over 250 volts". This seems to be commonly applied to systems under 250 volts.

Primarily on unfused raceways for 250 V. and less( metallic secondary conduits entering a meter socket, or a nipple between a meter can and disconnect. etc.)
