Does each piece of equipment connected from a tap in the service end box get a connection to a grounding electrode with a grounding electrode conductor?

I think the actual answer to your question is "sort of" of the equipment you are asking about requires a GE connection.

You can use a common GEC.

Not necessarily, you could have a common GEC with a tap to each service disconnect allowed, or you could run the GEC to a point in common like a splice/tap box that splits to each service disconnect. (Similar would apply to a building supplied by a feeder instead of a service)

Thanks but each does require a connection to the grounding electrode?
If the GEC is run to a point ahead of multiple disconnecting means - no, if GEC does not run to such a point, you need to hit every disconnecting means with a GEC. You can run a GEC to each disconnect or you can run a common GEC and make taps to each disconnect. The taps or individual runs to each disconnect only need sized per the conductors supplying the disconnect, the common GEC must be sized per the equivalent service (or feeder) conductors where there are multiple conductors per phase making up the supply.

If you do not go to a common point as kwired noted, this drawing may provide some guidance on one method:

Both of the last images posted are possibilities that I tried to describe. They are probably the two most popular methods you will run into to do a similar installation, but there are a few other possible variations.
