harmonics

I am in a plant with a 4000A gear. It is hot to the touch and vibrating. A phase is 194 degrees, B is 221 and C is 196. I have torqued all bussing in the past. I placed a power annalyzer and it is showing 43% on harmonics which I was told is high. Heat and vibration are side effects of harmonics. I cannot find a NEC reference that helps. Does anyone have good insight on this matter?
 
43% what harmonics?

1. Have you performed an IR?
2. Torquing a bad connection does not improve the connection!
3. Ambient in the room?
4. Air movement in the room?
5. Load on the gear?
6. Type of bus?
7. Could be induction issues due to board design or wiring layout.

And the NEC won't help with this.
