If the scope of the NEC excluded utilization equipment, I think 90.2 would read "Installations of electric conductors and equipment (other than utilization equipment) within or on public and private buildings or other structures..." But it doesn't.

I don't see it that way. I think the NEC does exclude utilization equipment, and my basis for that assertion is that it includes the word "installed" in 90.2. As I said earlier, placing a lamp on the floor and plugging it into a nearby receptacle does not comprise "installing the lamp." If you don't "install" something, then the NEC does not address it.
