heights of conduit up pole for 3 phase underground service

the electric company is going to set a new pole tomorrow and set a 3 phase 230v transformer. then go down the pole to an underground
then back up to main service. at what height do i need to be up the pole with pvc pipe?and what is on top of pipe weatherhead?

My poco has a very clear customer handbook. its your conduit on their pole, done per their details.
our poco doesn’t like to install weather heads, but we insisted

It's entirely up to POCO. We have one here that requires conduit all the way up the pole but they only want us to run from the building, throught the trench, and up one stick on the pole. They require us to leave the rest of the conduit and they will install it. Another POCO just wants a sweep from the building, they run their own conduit in the trench and up the pole.

I can easily see why POCO would not want anyone else climbing the pole all the way to the transformer to install conduit. Especially if the primary wires are live at the time.
 
Our poco has spacing requirements for the standoffs, one in the dirt, the other at 7 ft, so with PVC violates the spacing requirements. They don't want someone climbing the pole using the standoffs. We install the first ten ft of sch 80 and leave a coil of wire. We tie the wire up high on the pole

That’s odd they are knowingly in violation of NESC.
NESC Rule 217A2c
Standoff brackets on supporting structures shall be arranged so that there is not less than 8 feet between either:
1) The lowest bracket and ground or other accessible surface, or
2) The lowest two brackets.
Exception: This rule does not apply where supporting structures are isolated.
