I haven't encountered any in use, but #17 is what I think of when hexaphase is mentioned.

Three center-tapped secondaries with the center taps joined as, or to a common neutral.

Seriously though I think thats configuration available on gen sets.
As far as transformers Winny & Wayne are the only ones I have ever herd about it from:
