Does not the double pole GFCI protect against the problems listed?

it would if the current is coming from a GFCI and there is enough leakage to a grounded path, and if the GFCI is even functioning, but code does not depend upon the latter as this is why they also require the EPBG and equipment grounding around hot tubs and pools, and if the voltage is coming in on the service grounding from a service neutral voltage drop or a problem with the MGN then the GFCI will not detect this condition at all, so depending upon GFCIs is not always the best answer.
 
Correct, there is no exception for an elevated above ground wood deck that supports a hot tub.

This really needs to be addressed for future editions of the NEC.
