You would think in modern times there would be some type of safety bypass on the plumbing to shut the pump down and or relieve the suction if such an obstruction occurred.

It has been a requirement for many years that dual return ports be used to reduce suction at one for this reason.

As noted there has been changes not only to the plumbing but also on the grates. There are standards. Old pools with one drain can be retrofitted with an SVRS (suction vacuum release valve) and new grates. There are a lot of videos, stories, and info about the history.

It has been a requirement for many years that dual return ports be used to reduce suction at one for this reason.
And even then, most are not flush but rather they protrude from the surface and have inlet holes in both the face and the ides - less chance of total sealing and having the suction keep you from being able to pull away.
