How to come up with a rate?

I would like to know what you guys use to figure a rate and what is common practice to charge for,travel etc.What is a normal markup and is it on total cost or just materials. Started doing some extra work and would like to be competive and professional.

MD has a table that he uses for markup. Looks pretty good. It's on a scale where the customer pays more markup for little stuff and less for expensive stuff. Makes sense as you still have to to order and do leg work even for a trim plate.

If I recall correctly, he does not use that in his business, but it was presented as a model
